UKM application and BIOMETRICS Questions

Hello, I am applying with the UKM form and I am confused because it asks for all of my biometric information on the form, yet when I read these forums, many people say that they got a letter/email telling them AFTER they applied where to get their biometrics test in the USA so I am confused, do I leave that section blank? I don't want to have my application denied because I did not fill that part out. Also, is the fee for UKM only 80 pounds? Strikes me as odd because it is so low. All of the other fees are in the 100's if not 1000 pounds. Also for those who have applied from the USA did you send a self addressed prepaid envelop to get your papers back and if so which courier did you use? Thanks so much for all of your help.

Not sure which question exactly you are referring to. However, if you have never had a Biometric residence permit for the UK, then you tick 'No' to question 4.1 and continue to question 4.12. If that is not relevant either, then tick 'No' and continue to question 4.16.
I don't want to have my application denied because I did not fill that part out.
You must complete the sections that are relevant to you, even if it is a 'No'.
Also, is the fee for UKM only 80 pounds? Strikes me as odd because it is so low. All of the other fees are in the 100's if not 1000 pounds.
You only pay for the ceremony. The fee is low as UKM was introduced to correct the law that prevented British mothers from automatically passing on their citizenship, so charging 100's of pounds would not make sense. Form UKF (British father), is also only £80.

I left the section blank. They informed me to pick out a local USCIS office. They made the appointment and had the fingerprint and photo done at USCIS.
I didn't send a return envelope or postage. I used USPS first class. They sent back my originals DHL.

Another question, I must sound dumb but, on the application 1.2 is Surname (so last name) so where do I put my First name? I don't see a section asking for my first name. What am I missing here?

Hello
Maybe I am missing something but on 1.2 of the UKM application it asks for the Surname (aka Last name) but nowhere does it ask for my first name so where on earth do I write that? 1.1 is my title, 1.3 is other names? and on 1.2 it does not say "Surname, First name." So I'd love help please on how to fill in my name.

On line 1.2 I put my last name.
Line 1.3 Other names - I put: first name [space] middle name
