Hi All,

Am applying for a passport for my son. Whilst applying for his birth certificate I made a mistake with one of his middle names and had to send it back for correction. New birth certificate still have the wrong name but a note at the bottom with the corrected name.

My question is on the passport application form do I have to fill the "previous name" part of section 2?

I will say do mention it, and also mention it in Section 8 that it was a mistake and you have corrected it and send a Full Birth certificate along with the application.
