Previous relationships

Hi. I'm applying for UK passport (naturalisation). Im from EEA country (Poland). I have 2 questions about my marital status, and I'm not sure how to answer.

First, I'm living with my girlfriend for almost 5 years. She is not British citizen, she is also EEA citizen. At the moment she is not applying for the naturalisation. In the question "Do you have a husband, wife or civil partner" should I select Yes? We never registered our relationship in any office. Usually in a similar applications you can find common law partner. What I should select in such situation ?

Second question is about Previous relationships. The question on the online form is:
"Have you been married or in a civil partnership before or had children with anyone other than the partner you are applying to remain with in the UK? "

I was married since 2007 and 2013. The marriage and the divorce was done in Poland. Never any marriage document was provided in any UK office. Of course I was selecting "married" in any applications during this period. I don't know where my ex is now, we don't have any kids etc. Should I select "yes" and provide her information or it is only for people who had been married to UK citizens or in UK ?

I tried to find information about this, but can't find anything. Can anybody help?

Girlfriend doesn't need to be declared - not a formal relationship. Also there is no legal status of "common law partner/wife/husband".
Marriage - should be disclosed whether UK or not, if you don't know the whereabouts of your ex say "unknown".
