ILR+ Naturalisation+ Proof of address

Hello, I have 2 questions. Came to the UK from Eastern Europe in 1995, claimed asylum, was granted permanent residence in July 2005. As my birth country joined E.U. in 2004, did nothing with my permanent residence (ILR). Since 1995 Lived, worked and was on income support for a while when I had my children. Questions:
1. Is Apply for Naturalisation (AN) form is right for me?
2. Do I need to prove addresses for the last 5 years? Or I do not need proof of address at all? There is nothing about the proof of address on the new online form.

You were granted ILR under UK law rather than EEA regulation. So there is no need to prove you exercised treaty rights
You can apply under AN. Also there is no need to prove your addresses.
