Mn1 or Form T

Good afternoon
Please advise seniors.
I am in a process of submitting naturalisation for my sons who are born in Uk under section 3(1).
One of son is 11 years old now. He was born in Uk than went back to his own country for 2 years. So from age of 2 years he is living in UK.
I am confused about form. Which form I should fill Mn1 or form T for him?

Mn1 and it is a section 1(3) registration for a UK born child not a 3(1)

For submitting online application. I fill online referee form and for signing I download it and get it signed?
Can a friend working in NHS give reference?

Good afternoon,
Hi I want to ask about mn1 application. Is it necessary that professional referee knows child in last 3 years. My son started his nursery in January. His teacher is happy to give reference but she said if its necessary to know child for 3 years than may be she cannot do it.In new guidance of mn1 didn't find much information about referee.
Also I printed the form now but applying in 20 days time. Is it fine?
