Proof of EEA Dual Nationality

I searched the forums but did not find the answers I have been looking for.

Long story short, I am applying for Naturalization as an EEA citizen of Germany and Poland having previously obtained Permanent Residency Card. My question relates to required documents. Currently I am only able to produce a German passport to prove EEA citizenship but unfortunately I have no document proving my Polish citizenship. I have held both German and Polish citizenship since birth. My UK driving licence as well as German passport states that my birthplace is in Poland.
The guidelines are not clear whether I have to produce evidence of ALL the citizenships I claim. The online form lets me say that I do not have id card nor passport confirming my Polish nationality.

Is it therefore ok to send off the forms without any documents proving my Polish nationality?

You don't have to prove all your nationalities - one is sufficient - use the German passport as your proof of identity/nationality.
