If you are not married to a British citizen, then you have to wait till you have 1 full year on ILR.

Yes as long as you meet the physically present in the UK exactly 5 years ago requirement too.

there is no 'general' link.

It is part of the application process once you have booked your UKCVAS Sopra Steria appointment. You upload to UKCVAS website!

Online form doesn't give you the option to sign and print only AN from print out

There is a referee section you have to complete. Once you have finished the form, you get the option to download.

In fact, you can get referee forms in advance, before submitting application - they are blank firms anyway (you do not get forms with you referees data already put on in!)
Here:

https://visas-immigration.service.gov.u ... on_MN1.pdf

Referees can sign before or after you submit applications.
After submitting tour application you will gave to upload scanned references together with other supporting documents.

Yes.
2) Proof of living in the UK - 5 Years bank statement, 5 years P60, employment confirmation and payslips
Not required. You only need your passport(s) covering the whole 5 years.
3) proof of freedom from immigration time restrictions- ? is that my permanent residence card
You have ILR not PR (which is issued under the EU rules). It is your ILR BRP card.
4) The degree certificate to prove the level of English language required - Degree Cerficaticate
Yes if it is a UK degree.
5)Two referee declarations - Signed document from the referees
Yes.
6) Current Biometric Residence Permit - Yes I do have it
Yes, per number 3.
Finally, Do I need to upload my birth certificate and assist me on question 3
No. Birth certificate is not required.
