re-declared application form

Hello,
I have been informed with copy of my application from Home Office regarding the mistake of my application that I was not in UK 5 years ago based at the same date with my application date. And I was suggested to do the following.
"However, if you wish to send the attached copy of your submitted application form back to this address,
we will be able to continue our consideration. Your application date will be the date that
your re-declared application form is sent back to the email address."
But I am confused a little about how to re declare the form. Should I print and sign the application form with new application date or I will just reply them with the attachment for re-declaration?
