Naturalisation Form - Confusing Questions

1) Do you have a previous visa, entry clearance or leave to remain where you had to prove your knowledge of English? Tell us the previous visa, entry clearance or leave to remain you had:
-> Should I include ILR, FLR(M), and SET(M) Entry Clearance, all three? If yes, do I give the reference numbers for each application?
2) Have you had any other work over the past 10 years or since you came to the UK?
-> Only lived in the UK for 5 years with 1 job. Do I also need to list jobs taken up outside UK before moving here?
3) Proof of Living in the UK for the qualifying period ( 3 years )
-> Is passport sufficient?
Any advice would be appreciated. Thank you.
