Citizenship on the basis of tier2 general 5 years route

Hi all
I have few question for my British citizenship, thanks in advance !
I m going to apply on the basis of ILR and ILR I got on the basis on tier 2 general for 5 years but I am in this country for more than 12 years!
1. Have you made any previous UK immigration applications in the UK or abroad? - do I need to list all for 12 years or for 5+1 years only?
2. Have you had any other work over the past 10 years or since you came to the UK? - do I need to list all for 12 years or for 5+1 years only?
3. Do I need to provide 5 years or 12 years council bill?
