Proof of living in Uk & referee form

I have got couple of questions regarding my nationality application
1) how can i proof living in Uk for 5 years ? P60 ?
2) Referee form where I have to put my picture do they have to write anything at the back of my picture ?

1) Passport stamps only.
2) Follow as the form say.

Can you also please let me know about trips outside Uk is from you entered Uk first time or last 5 years ? I provided details of trips made in last 5 years only
