Doubts in AN form

Dear Members,
I am filling AN form (5yr T2G + 12m route) for BC and have a doubt in following question:
"Details of trips outside the UK" -
Do I need to put in absence details for the last 12 months only or include the absence details provided in ILR form (5 year absence history?)
Due to Covid19, I do not have any absences from UK since Jan 2020.
Appreciate your response for this.

For the following question in AN form:
1. Have you made any previous UK immigration applications in the UK or abroad?
Should I answer yes and provide details of ILR and T2 extension application details?
2. One of my friend who is also a dentist is ready to give reference , is it acceptable as professional reference?
3.I am also applying for naturalisation of my children at the same time, Can I use the same referees for all of us or need to get separate referees for all?

I have completed the application form. In the required documents section it is stated that:
Proof of living in the UK for Mr XXXXXX for 5 years if applying in your own right or 3 years if applying as the spouse of a British Citizen.
Also mentioned:
Bank statements or household bills are not suitable proof you have been living in the UK.
What are acceptable proof of living in the UK?
