MN1 form Child Consent form confusion

Hi,
Hope you all well.
i got 3 pages of Consent to the application where on first page father and mother will sign which is ok.
on second page Guardian’s consent
my question is do father or mother will fill and sign this page?

Guardian section would not be applicable as parents have signed their section.

Thanks but when i look into the checklist i found following
A letter of consent from Mr Baby Name parent(s) or legal guardian(s)
The letter must:
give their full names and addresses say that they agree to let you apply for a British citizenship
If you have more than one parent or legal guardian, they must both sign the letter and confirm that they agree to what it says. If you only have one parent or legal guardian they must explain why the other parent is unable to consent e.g. whereabouts unknown, no contact since MM/YYYY.
the page one parent section doesn't have address ?

child live with both parents no guardian.

yes but the part one does not have address. where we will add address or it doesn't need

just another question just book for bio, in document they are asking for child identity whereas we dont have any travel documents for child.
what would be the child identity at biometric is that birth certificate or child identity not required as responsible adult identity is enough?

Hi, so what document did you end up using for your babys identity since you dont have any travel document?
