Proof of living requirements

Hi guys and gals, I need bit of help. I have been living in UK from last 11 years and am on ILR after 5 years of Tier 2 and applying for naturalisation and bit confused about proof of living. I have below questions and would be grateful if someone can help.
1 - Do I only scan pages with entry stamps of last 5 years or all pages in passport covering last 5 year?
2 - Do I also upload old visa's as proof as well?
3 - Do I have to provide evidence of of my first entry in UK?
4 - Do I have to provide evidence of all old visas?
