Employment details employee/self-employed doing both

Hi,
I am filling the form for naturalisation.
There is a section to say about my employment details.
I am a employee of a company and also I am self employed as a professional photographer from 2019 doing on weekends.
I have done my tax returns for my self employment for 2019-2020, need to file for 2020-2021.
Do I need to say about my self employment as well? Then is it ok to just add my self employment as another job with my employee name as my photography name?
Do I need to select both section as asked below? But this section is for my current employment.,
