Citizenship application

Dear All,
I need advise i want to apply for citizenship. I would like to know i had issues with credit card late payment and went default on my credit history which is cleared now on my credit history. as its more then 6 years now
do i need to declare it on the form
also i had 3 point for using phone whilst driving back in 2012 i dont remember the exact date.
do i need to declare it on the application.
