LITUK Test and Disability

Hello all,
I was in the process of doing my LINTUK Test and even booked and paid for an appointment to do the test but sadly I had a catastrophic car accident which not only resulted in missing the test but also most importantly in life changing injuries.
I wanted to fill a Waiver request for the knowledge of language and life in the UK requirement: medical opinion. I can speak and write English fluently. However my medical condition now limits me greatly as I am not physically mobile, can drive and can only walk a few steps, aided. I have also other medical conditions as well as severe psychological issues, including PTSD and cognitive impairment due to my long stay in ICU, in a coma. I suspect I would be able to do a Video call to demonstrate my ability to communicate in english, but not sure if this is possible. With regards to the LITUK Test I do not have the mental focus and cognitively to learn the material.
As I am under the care of several specialist consultants as well as my GP, who is fully aware of my situation, who would be best to help me with this Waiver? My GP, my Psychiatrist?
Any advice would be most welcome. Thank you
