Bo need for separate application forms.
When you you have completed the AN online applications for the adults you can add child under 18 and the system will adjust to the MN1 format for registration. Note that each applicant will still be assessed individually.

Hi Team,
1) Need to apply Citizenship for myself , spouse and 2 children's one under 18. Do we need to apply individually or include everyone in my Application form and pay total amount..

2) Do we have any format of referee declaration.

Thanks,
RP

As I answered last year, you can all apply on the same online AN form, add spouse and children after you have completed your application. Or you could do it all separately, your choice. You will have to pay the individual fees for each application, or in total.

2) Do we have any format of referee declaration.
Form for both adults and children: https://visas-immigration.service.gov.u ... on_MN1.pdf

Hi,

thanks you..

For the below document Ecctis letter that confirms the qualification meets or exceeds the recognised standard of a Bachelor's or Masters degree or PhD in the UK AND confirm the level of English to which the
degree was taught or researched can we provide the ESOL- B1 certificate as evidence.

2) In the referee declaration form whether the date has to be prior to application submission date or any date.

Thanks,
Rp

If you had to prove English for ILR then you can rely on that - select the relevant option.

Can be dated before or after submission date.

Thanks,
