Name in which you made the application

I am filling the form for Apply for naturalisation as a British Citizen that there are two questions i would need your assistance please

1. "Name in which you made the application"

I am planning to insert as following , is that correct ?

[Mr Name Surname] [Tier 2 ICT Long Term Leave to remain]

Is above format is ok , or should I just insert my name without Visa type mentioned?


2. "Enter your reference number if known"

I insert the BRP number i got as i do not know what reference number means , is that fine?

1.Just your name.
2. BRP # is fine, but it's an "if known" field.

Thanks , is that reference number something on appointment letter such that below?

UAN: 1212000xxxxxxxxxx/00

Otherwise i will keep the BRP referance no's there

Can't comment on UAN format, if in doubt use BRP#, but not an issue, it's an "if known" question.
