References for child

Hi,
One of my child is not going to nursery, I'm planning to give same references given to me. And I can prove that the child is in the waiting list to join, hence, I couldn't provide other references.
But for another child who is in year 4 , I've not asked school teacher because we don't know whether she is willing to give her full details and I'm also not comfortable asking as we hardly speak to her.

I'm planning to give the same reference that I'm giving to mine. is this going to be a problem?

As the page 10 of the document says :
For child applicants at least one of the referees must be a person who has dealt with
the child in a professional role such as a teacher, doctor, health visitor or social
worker. Where a child cannot provide a referee who has dealt with them in a
professional capacity and has provided documents to show that they have attempted
to do so, you can accept 2 referees who meet the criteria for referees on adult
applications.
A per above rules, if I can't provide then I can provide an attempted to provide proof!!
So, my question is how do I prove that I've attempted to get reference for the another child who is in year 4? If anyone has any experience, if ther is no issues giveing and need not worry about the references, I would just give the same reference as Mine and be done with it.
