Name in which you made the application

Hi,
I am applying for British naturalisation for myself and my family. I first arrived in the UK in 2018 on a Tier 2 General work permit valid for three years, which I later extended for an additional two years. In both cases, I submitted a single application where I was the main applicant, and my wife and family members were included as dependents.
For my wife's citizenship application, under the section "Have you made any previous UK immigration applications in the UK or abroad?", there is a field asking for "Name in which you made the application."
Should I enter my name (as the main applicant in the original visa applications), or should I enter my wife's name? The appointment receipt shows the same reference for both, where my Unique Application Number (UAN) is xxx-xxxx-xxxx-xxxx/00, and my wife’s is xxx-xxxx-xxxx-xxxx/01.
I appreciate your guidance on this.
