Form AN - Questions

My spouse is filing form AN as she has completed 5 ( Skilled worker) + 1 Year ILR duration.
I have need some guidance on below questions in the form.
1. Have you provided evidence of your English language ability in a previous application?
I think She need to provide earlier NARIC assesement and degree copy done in 2018 year.
2. Have you made any previous UK immigration applications in the UK or abroad?
She has made 7-8 application in past as Tier 2 dependent ( during that time) and SKilled worker. Also on her old passport . Do i need to provide all details of these visa applicatons ?. The dates may not be accurate ? Is that okay ?

Answer YES and provide that.

Thanks. For referees , We have filled details of 2 doctors working in NHS ( both british citizen) . One is a family friend. Is this okay ?

As per the document checklist , below documents needs to be submitted.
.1. Colour copies of all pages of the passport or travel document for xxxx from xxxx . This
includes blank pages. The copies must be in page order, and clear enough so all text can be read easily.
2. Proof of living in the UK for xxxx for 5 years
What i need to provide for point number 2 ? Is P60's for last 5 years enough ?

A copy of the stamped pages on your passport for the last five years will be your proof of living in the UK.
P60s are not required.
