HELP FILLING IN FORM MN1 section 1

Hello
I'm applying for registration of my son, but I'm a bit confused on section 1.5 :oops:
His name on birth certificate is: MATIAS MAY SORENSEN
Matias is his first name
MAY is his dad's surname
SORENSEN is mother's surname(mine)
So, How should I fill section 1.5/1.6/1.7
I'm confused 'cause 1.5 is asking just for surname

Hello Sailor....;)

I'd put Matias May-Sorenson, ie double-barrelled surname but at the end of the day so long as no fraud is indemted you can put whatever you like.

In section 1.5? this is asking just for a surname/family name

I dont think so. Neither the form or the guidance note MN say anything about that.

Do you intend for him to use that complete name all the time? Would he go by Matias May or Matias Sorensen as a shortcut? If not, if he will always use both, I'd write "MAY SORENSEN" as the surname.

Question I would ask you is 'What is your son's surname?' You have told us what his dad's surname is and what his mom's surname is.

Also I hope you are using fake names by the way in your example

As I tried to explain, he's got those two surnames on his birth certificate, My confusion is because the application is not asking for his first name, so where should I put it?

If on his birth certificate his surname is May Sorensen, then you put that on the form. First name and middle names will be in 'other names'.

you say he has got 2 surnames on the BC do you not mean he has 2 names as his surname. Have you considered putting a hyphen and making it 1 name ie May-Sorensen or Sorensen-May, cause if you dont then what might happen when he is growing up is that he will most likeyl end up being called Mr Sorensen, with May being a middle name or vice versa
