Require Documents and Fee for Citizenship

Hi All,

I will be applying on the basis of 5 years continuous residence with 1 year on ILR. What are the supporting documents for the Citizenship Application? I found the following from the Application form,

1) Life in UK test certificate
2). Passport with ILR stamp and to prove the continuous residence for five years
3). Driving License (Used for Life in UK test, so need to provide the same for identity)

Do I need any other document, P60 etc?

What is the mode payment for the application fee? Can I pay by Credit / Debit card at NCS? or Do I need to enclose a cheque payable to HO? NCS has mentioned in the confirmation of my appointment letter that the application fee made payable to 'The Accounting Officer, Home Office'.

Can you please advise me?
