QUESTION ON FORM AN 1.23-1.30 DETAILS OF PARENTS?

Please can someone help me with this question regarding details of parents.
I do not know my parents date's of births as my mother is deceased and I am not in contact with my father at all.
Also can my work freind be a referee as I have worked with her over 3 years. Also she is the sister of my mother in law's husband?
would she be classed as a reletive?
Also my mariage cert is translated into english as the original is in Thai lanquage.
Will this be ok?
