Few silly questions, hope can still help!!

I am filling the form BOTA to register as British Citizen, got a few silly questions.

(1) It asks to put previous HO reference, so I guess it is the reference in my last year ILR acknowledgment letter? Start with IR……??
(2) Previous addresses, I don’t know the exact date of moving, can I write down eg From JUNE 2010 To JUNE 2011??
(3) Tax office reference, where can I find it?
(4) In the declaration page, should I tick all the boxes including the box – ‘the Insolvency Service (England and Wales) etc etc?
(5) Also in declaration page, one box is about declare you have special circumstances. If answer is no, I just cross and delete the whole statement? (as it said ‘delete if not applicable’).

Sorry I know some questions may be very silly but I just want to make sure I fill the form 100% correctly.

anyone may able to answer some of above questions? even some but not all, will be a great help.

(1) Left it blank on our form
(2) I was told you had to put an exact date but ultimately if it is out a little bit then that isnt an issue
(3) If you are an employee your HR/ Payroll department will be able to tell you which tax office it is
(4) You must tick all the boxes if you want them to be able to process the application
(5) 6.7 you would state none. The Delete if not applicable bit above refers to 6.4 which is the declaration you've enclosed the necessary documents. Cant think of any circumstances that you'd have no document to enclose but I guess there could be some?
