Dependent ILR Query

Dear Board Members: Would be really helpful if you can answer my query below:

Query Background:
>> My wife entered UK on 02/12/11 on a Tier 2 ICT PBS Dependent: 12 month visa
>> Visa got extended in 2012 for two years and is valid until dec'2014
>> I got my ILR approved in Mar'2103
>> We didn't get my wife's visa converted to FLR at that point in time as it would have brought her under the five year rule
>> I changed jobs in sep'13 and I think that my employer may have notified Home office regarding curtailment of my sponsorship(this didn't affect me as I had already shifted to ILR by then)

My Query:
I am now planning to apply for my wife's ILr under the 2 year rule as she came before July'2012, my queries are:

A/ Which form do I need to use Set-O or Set-M
B/ Will my wife's PBS dependent visa cause an issue as the main visa holder i.e myself has shifted to ILR which has left the tier-2 visa ir-relevant and hence this may have caused the dependent visa to be curtailed
