Quires on SET (O) application

Hi members,

i have some questions on the SET (O) application:

1. in section 6 (6.1) what date should be entered here is this date of my first HSMP approval or first entryclearence date as student?

2. in section 6.2 should i need to metion only last 5 years of out of UK dates for example if i am applying for ILR on 23 Dec 2013 shall i just mention 5 years back wards i.e until 23 Dec 2008 or do i need to mention for date i came UK as student??

3. in section 7.11 do i need to put the period form my first entry clearence i.e in Sep 2005??

4. i was away for less than 6 months continously do i need to mention this in the table below 7.11 section??

5. but i was out of UK in 2008 for more than 180 days not continuosly went away for 5 months came back and then went for 15 days once and 18 days once.

i would really appreciate if any one can answer.
