ILR Application for Wife(PBS Dependant)

Hi, requesting for few clarifications as I need to submit my wife's ILR application this week.
My history : Arrived in UK May 2010(T2 ICT) -> Converted to T2 General Aug 2014
T2 General Extn May 2016 -> T2 General Extn Jun 2019 -> ILR Oct 2019(T2 General 5 year continuous residence)
Wife's history : Arrived in UK Mar 2015 (T2 dependant) -> extn May 2016-> extn Jun 2019, expiry Jun 2020 (PBS dependant)

My questions are
1. I think she needs to fill in SET (O) form?
2. "Which visa Mr. XXX (myself) applying for or been granted." - I don't see any option for ILR to choose from the drop down. Shall I select T2 General here?
3. "Do you have one of these ref no for Mr XXX(myself) - VAF# ,GWF#, none" - is this referring to my original application ref before I entered UK in 2010 (T2 ICT)?
4. "Do you have COS ref no of Mr XXX" - is this referring to the last COS # I had from previous employer before applying ILR(have switched job after ILR)
5. "Does the period of stay on which this application was based include any leave granted since 11 Jan 2018?"
I'm confused here whether to select YES or NO. In case of NO It's asking travel histories only since Jan 2018.
In case of YES it's asking her entire Travel history -> In this case whether copies of all her old and new passports will be sufficient as evidence? She was not in employment during entire duration of her 5 year stay in UK so letter from her employer will not work.

Put your BRP number

Yes, enter the old COS info

You should select YES. Only her absences from Jun 2019 onwards have to be declared as PBS dependants have no absence limits (during their entire period of leave) if such leave was issued before 11 Jab 2018. Employer letter is not relevant for PBS dependants

Are you sure its asking for COS number? or the employer license number?
