Applying ILR for dependant

Hi,

I got married to my wife in April 2019 via court marriage who was in T2 skilled visa and below is my visa timeline and route:

1) First visa - August 2019 received dependant visa printed on passport with expiry of one month to collect BRP with visa expiry for 2 years.
2) Second Visa - Visa renewed in June 2021 for 3 years on Tier 2 dependant
3) Third Visa - Wife got ILR in Jan 2023 and Visa was renewed to spouse/ partner leave to remain

I have been married for 5 years and stayed within the UK for the last 5 years and need to apply for my ILR and want to know which form i.e., form O or M should i apply? or anyother form if I have missed?
