SET (O) Form Questions

Hi all, i have my appointment on 27th of this month. Can somebody answer my following querries , trying to complete Set O form. Thanks in advance.

1.Question 1.10-- Home Office Ref. No. -- Which one i should use? one on initial HSMP letter or one on Tier 1 approval Letter.

2.Question 7.1 -- Conviction ---I got a Bus Lane Contravention fine in 2007 ,but it was my dad who was driving my car. i was not even in uk at that time.I Went on bussiness trip. Anyhow i paid it ontime after coming back & no court was involved. Do i have to mention it?

3Question A5 --- On pg 16 in Employer Detail , the first column, Start & End date? Is it the start & end Date of my employment in this
Company or start & end Date of Earning Claiming Period.

4Question ------ Regarding qualification , they are asking for Grade. There are none for me as they dont give any grades on degree in my
Country.

5Question -- On pg 35 --- Question 13 -- Biometric Details ---I have got both under 16 ( my son & Daughter) and over 16 dependent (My Wife).

6Question --- Declaration on pg 36 should be signed by my wife as well ??

1) Home Office Tier 1 reference number
2) Conviction by court is to be declared. Parking/bus lane fines are not conviction
3)Start and end date for earning period claimed
4)Leave it blank or write NA
5)Tick less than 16 years. Mother will accompany children and also sign item 17

Q6 there is no space for any declaration or signature
