SET(0) form queries - simple ones hopefully

Hi,
I needed some help with filling in the SET(o) form so any advise is appreciated.
Section 5 - Question 5.3:
Are you working in the UK?
If so, what is your pay each month after income tax and other deductions?
I have answered yes that I am workin in the UK. However, as I am the director of my own limited company this is a difficult question to answer in a small box on the form.
I get a monthly salary (a fairly low amount - lower than my monthly rent which is the previous question) and a dividend (which varies by month).
What should I show here and is there any link required between this figure and the detailed earnings that I will present in section A.5?
Section 6 -Question 6.3
When did you last enter the UK?
Probably quite a silly one to even ask for confirmation but nonethelss - I am putting in the date when I last entered the UK from a recent short vacation. Am assuming that this has no bearing on any other date like first entry or anything else.
