FLR (M) Questions - Urgent

Hello everyone,
My visa is expiring on 18 August therefore prompt responses will be appreciated.
Background:
I came to the UK on 27 Sept 2010 on Spouse visa but my visa is valid till 18 August 2012. I am eligible for FLR (M) and not ILR yet.
Questions:
1 - I am applying under the category of Spouse of a person present and settled in the UK and a biometric immigration document. But then it asks:
Is this the first time you have applied for a visa or extension of stay in one of the above categories (including previous leave granted as a fiancé(e) or proposed civil partner) with your current sponsor?
Shall I answer yes or no? As this is the second time I am applying for a visa but first time for an extension.
2 - How long have you been living together in a relationship? Answer: 2 years.
Do I need to write addresses and the time we spent in those addresses?
3- Is 7A applicable to me?
4- Please give the details specified below in each case. If you need more space, continue on a separate sheet and enclose it with this application form.
I don't remember if they were taken. Does anyone know if they were required in 2010 on spouse visa?
5 - Home Office Reference, what is this? I cannot find any letter or document by Home Office that mentions reference.

You probably haven't received a reply because you've left members to guess what most of your questions refer to. For instance 7A. Without reading through form FLR(M) this can't be answered.
You may want to seek legal advice on whether a postal application for ILR would be accepted (as you appear to have only a 7 day gap) instead of applying for FLR(M). By the time the application for an extension is processed you'd probably have reached your qualifying date. You could submit a letter asking for the case worker's discretion to issue ILR. I'm not legally qualified and this is just an opinion, which is why I suggest you seek professional advice from an OISC registered Immigration Advisor or a solicitor registered with the Law Society, specialising in Immigration law.

Hi newbiepk,
Can I ask under what terms you got your initial spouse visa? my wife came to the UK as a tier1 dependent and now that I have ILR and her visa is expiring we tried extend it through FLR(M) as my wife came to the UK 7 months ago and she has not spent 2 years to be eligible for ILR. When we tried to extend under FLR(M) we were advised that my wife has to extend the visa as a PBS dependent even though I'm not on PBS anymore. This may be a completely different case to yours and if so ignore it. I'm just raising it here may be if a moderator sees this he she would clarify this to me

I am going to ask the aforementioned questions again. I think I posted them in a hurry.
My questions related to FLR M are:
1- Section 2: Category I am applying under
I came on Spouse visa and therefore I am applying as Spouse of a person settled in the UK.
2.2 asks Is this the first time you have applied for a visa or extension in above category? I think answer should be NO as I came on a Spouse visa.
2.3 asks reasons for re-applying under above category. I am applying because I didn't complete 2 years and feel short by 12 days. Does it mean I choose option: I delayed my travel to the UK?
Also I have following questions related to ILR:
1- If I write to the UKBA before they have made decision on my flr that I am now applying for ILR. I just need to submit to Life in the UK test certificate in addition as all other documents are same for FLR and ILR?
2- There is a question: Have you lived with your partner permanently in the UK in the last 2 years. I have but I went to my country for holidays alone. Do I need to mention it somewhere?
