wife ilr application as spouse.....Help

please help confused
my wife got the spouse visa frm pakistan 26 aug 2010 valid till 26 november 2012 she travelled to uk 19 oct 2012 she had her finger prints taking at the time of appling spouse visa
we will be booking peo appoinment around 21 sep 12 to 30 sep 2012 deping on availibility with (set m form) i guess that is 28 day before the first time she came over to live her in uk.... is that correct??
second we will be filling the form set m and making the payment at the peo does she have to fill in seperate form for biometric form??
