Which form do I need to fill if I’m not self-employed ?

Please someone help me !!!
I’m BULGARIAN citizen. I have been living and working in the United Kingdom since 19th of November 2004.
I have Registration Certificate (Blue Card) since 17th of May 2007.
I have passed Life in UK test on 13th of October 2010.
I’m worker, not self-employed. I have been working constantly for all those years, since 19th of November 2004.
I would like to apply for ILR (Indefinite Leave to Remain) or Permanent Residence.
Could you please tell me, which form do I need to fill if I’m not self-employed, to apply for ILR or Permanent Residence?
