ILR having been on HSMP (not judicial review)

Hi,

I am booked into Croydon next week for a premium ILR. Here is my Visa history:

Working Holiday: 3 May 06 - 23 Aug 07
HSMP: 24 Aug 07 - 24 Aug 09
Tier 1: 27 Jul 09 - 24 Aug 12

2 questions:
1) In section 3 of the ILR, am I applying under section A - Tier 1 (General) Migrant or another category?

2) What HSMP related docs do I need to provide where it asks "For the period before you were granted leave as a Tier 1 (General) migrant. During the HSMP period I had regular P60 income and also contracted.

Apologies for the stupid question but I am confused & am so close to my visa expiring that I only get the one shot at it. Thank you so much in advance for the help.
