My situation is HSMP (2 years ) + Tier 1G (3 years) and pending to apply for PR this year. I will go for PEO. My understanding of the procedure is:
1) Apply for an appointment online
2) Prepare all the documents and go to Home office on the appointment day making the application with all the documents including applications forms, etc.

But after I register for an account in UKBA. There is a sentence saying 'you do not currently have any transaction. you should not book an appointment unless you have already submitted an application and you have been informed that your service requires an a ppointment.'

Do I need to send out the application form online? If so, how?

I have exactly the same problem. Does anybody know if we nee to obtain some sort of reference or something from HO that will allow booking of an appointment?

dont worry guys that's normal. When you create account you will see that no transaction means you didn't book any appointment. Just click book appointment leave the date blank choose your peo and keep on trying until you get some thing ( who knows when ) but u will get it around 12.02 am for sure..
