ILR Query Driving Offence - Urgent

I got HSMP on 19-10-2006 which comes under Judicial Review. I came to UK in 2007 and now due for my ILR.

My scene is I was stopped by police in November 2011. I was driving a Car on a Provisional Driving Licence supervised by a person who has a full UK driving licence but less than a year. Police stopped me and asked me some questions. Then I was instructed by police officers to submit my Provisional licence with counterpart to the nearest police station within 7 days which I did the next day. They fined me 60 pounds as well which I paid online within a week.

After 2 weeks I received my licence and counterpart and on counterpart it says the following details:

Convicting Court Code: 2665 FIXED PENALTY

Date of Conviction: ------

Offence Code: LC20

Date of Offence: 12-11-11

Penalty Points: 03

Please note that i never been to any court and also never received any summons.

My Query about the above details:

1) Is the above driving offence comes under the category of Criminal Conviction?

2) Do I need to disclose that while writing my details in SET (O) form for ILR in Section 7 (Question 7.1) ?

3) Can any of the honorable members who recently got ILR on HSMP JR 5 years Category provide me a list of documents i should finally attach with my application for me and my 2 dependants (My Wife and Son)?

Need an urgent help from this loving forum. :) :) :)

This is not a criminal conviction but u have to disclose it,,,tick ''no'' at the criminal convition section of set o form and attach a separate sheet on plain paper under the heading of ''additional information'' and give detail of the incident.

Dear mschauhan, do what safdar is saying.

Also take a print out of the copy 'criminality requirement for settlement' from the uk ba website where it mentions FPNs are just offences and not criminal convictions.

Also read new rules 13dec2012 onwards. This requirement is being taken out entirely.

thank u very much for replying my queries. can anyone provide me the list of documents i should send with my Application?
My HSMP arrpoval was in Oct 2006 so I think i am exempt from Knowledge of Language and Life in Uk test+Maintainence fund under Judicial Review. Can anybody confirm it?
Should i send bank statement and salary slips of 3 months or 1 year ?
