Apology as it may be a very silly question. Just now filled up the Set(O) form and found there are only 4-5 details to be filled for dependant. Bit surprised for lack of details here as in Tier 1 there was a separate document for dependant.

So, just want to confirm that I am not missing anything and there is no separate form to be filled up by the dependant for Set(O) application.
