ILR Queries please helpHi

hi
can you please advise me on my queries.
1. as i am appearing myself in PEO office so do i need to fill payment details column because i am paying by my credit card. or shall i just leave that blank.
2. on last checklist page there are two option A & B
BANK STATEMENTS - How many ? as i have got two jobs and i am showing 12 months bank statements for both ---- please advise what should i put
pay slips --- i have got 12 months payslips for both--- i don't understand what should i put as they have asked -- how many ?
