My first HSMP Visa start - 17/04/2008
First UK Entry - 14/05/2008
Tier-1 (Gen) Extn in 2010
UK Visa expiry - 17/04/2013
5 Year completion from first entry - 14/05/2013
[No break more than 2 weeks from UK in last 5 years]

So my queries are assuming I am eligible for ILR are ..

Q1. When is the earliest I can call for a PEO appointment ?
- 42 days b4 my Visa expiry ie 17/04/2013
- 42 days b4 my 5 year completion 14/05/2013

Q2. When is the earliest appointment I can seek..?
- 28 days b4 my Visa expiry ie 17/04/2013
- 28 days b4 my 5 year completion 14/05/2013

Q3. Due to tight time line is it worth to seek an extension before applying for ILR..? Because if answer to Q2 is second one then I have 2 days tp apply for ILR..!!

28 days before 17/04/2008 is the earliest you can apply for ILR at a PEO. So 20th march is the earliest you can apply.
Appointments are released 42 days in advance. So you should start looking to book appointments by 1st week of feb.
There isn't any need for 2nd extension!
