Help to fill ILR form

Please can someone help me in answering these questions? I am applying for ILR very soon and I have been here since 2005 and currently on Tier 1. I travelled outside the country for a month in 2007 before I got HSMP and I have not left the country since I got HSMP till now. Hence I am a bit confused in answering the following questions:

6.1 When did you first enter the UK?
- is this the date I got my HSPM visa or the date I arrived in this country (2005)?

6.4 When did you last the UK?
- Is this the date I first came into the country or the date I came back from a month leave in July 2007?

7.11 How long have you lived in the UK?
- When do I start calculating this please?

7.12 Please state what ties you have with
- Can I just say Brother/Sister or I have to give specific names?

On earning period:
Do I have to give two periods for earning if I am combining salary with self-employment e.g. salary ( 01/01/2012 – 31/12/2012) and self-employment (04/04/2012 – 31/12/2012)? OR should I just use one period since the period covers the two?

If your HSMP was issued in-country
1) it would be the HSMP issue date.
2) this is the last time you entered the UK, not first. If this was in July 07, then enter that date.
3) What would be relevant here is how long you have lived in the UK under your ILR qualifying immigration category. In your case this would be HSMP/T1G.
4) Brother & sister is fine, names are not required.
5) It's just one period that covers the two. But list each source of income separately when stating the actual earnings under the relevant section in the form.

Goldenboy, first of all i wish you all the best, i am interested in your case as we are both in the same boat. i am on I tier general and i got two source of income salary and self-employment, do you know if its possible for applicants with two source of income to apply for indefinite leave to remain under premium service? If yes what PEO do think we can book our appointment with. Can someone advice please.

Yes, you can. Please check the UKBA site for specific info. As I recall, Glasgow PEO do not accept in-person SET(O) applications for self-employed applicants. Additionally I have heard of cases where a same day decision was not given at Cardiff PEO, when self-employed earnings were part of the application. If you use the search feature on this website, you will find many experiences posted about this subject.

Thank you for wishing me well. It is possible to apply for ILR under premium service at Croydon. A friend of mine did his (salary+self-empl) last year and he was successfull. I wish you the best as well.
