dependent visa under category HSMP JUDICIAL REV. T1 general

hi, plz can u help me asap.
I apply hsmp visa by submitting initial application in oct. 2006. application was delayed and refused. I went in appeal and finaly i got approval letter in july 2008 and got 3 years visa (Tier 1 general) in Nov.. 2008. In 2012 i had applied for an extention and ukba considered my case under judicial review policy and gave me 2 years visa (Tier 1 general). My ILR is due in dec.2013.
Now i want to apply visa for my wife and two daughters (5 & 7 years of age). Plz can u tell me that
1. what are the relevant form
2. fee of each application.
3. my weekly wage is around £315 after tax while i am payin £500/- house rent.
4. what are supporting documents i need to apply their visas.
5. finaly if they get visa and come to uk, can i apply ILR for my dependents in dec. 2013 with my ILR application.
plz help me and i must say thanks in advance.
