ILR ICT Salary query

Hi,
I am on Tier 2 ICT visa and booked appoint in 3 weeks time for ILR. Can you help on the following queries please.
My salary consists of salary in India + allowances in UK.
1) My allowance varies in last 12 months, but meets the salary criteria. What's the monthly amount should I declare in the Set (o) form - section 5? is it average figure or current month allowance?
2) Also should I include Indian salary? if yes, is there a standard conversion rate from INR to GBP?
3) My payslip contains part of the allowances (for last few months) and remaining is reimbursed to bank account on actuals. Should I declare total amount or figure mentioned in the payslips?
4) Should I request my employer for a letter summarising the reimbursed allowances? Though amount quoted in payslips alone is meeting the criteria, I am unsure if CW may question, as recent payslips show less amount.
