ILR As A Long resident

Hello,
Could someone give me wright information about ILR as a long resident with dependent.
My 10 years will be complete on February 2014.
when i apply ILR with my wife please some one tell me which Form should i file. Is SET (o) or SET(LR). My wages is £18500k and is there any 90 day maintenance fund, do I need to show in this category.
