Change of name with ILR need help urgently

Hello
I need help in the following :
Been in the UK since when I was 12 .
Was granted DLR back then then I was granted ILR 5 years ago .
I have change my name by deed poll .
Do I have to apply for a new stamp or biometric card with my new name ?
I have been convicted twice and sent to prison for driving offences (reson I can't apply for citizenship )
Will the UKBA refuse my application for a biometric card with my name on it ?
