Set (DV) Appeal Grounds

Hi
finally had received reply from UKBA'
Refused with following Points.
1: Not Subsisting in the UK
2 ; Medical report not referring to Partner.
3 : Break down reason is not Domestic Violence.
Please help me How to make appeal . I wanna appeal on my own.
as i didn't mention any thing to the doctor as i knew they would call police. so there is no any record.
