Urgent questions about SET(LR)form

Hi.
I will be posting my application for indefinite leave to remain in the next two days on the basis of 10 years long residence. I have completed set LR form and will be submitting 2 passports, Biometric card and Life in the UK test only. I've got a few questions:-
1) Is SET LR form, VERSION 4/13, the correct form for my case. I was a student for 6 years, then PSW, then Tier 2 general.
2) I will be paying by postal order and I have ticked section A6, postal order..however, It then states " go to question A19" which does not exist? Is there question A19?
3)Is it necessary to send a cover letter as well?
