Set O Form section 9 evedience for regulation of accountant

Hi all,
If anyone can help me with regard to new form of SET O. In section 9 it has been written that on earning section that evidence of the regulation of accountant body must be provided.
what is that mean? if accountant letter head is already clarify its registration number, still have to provide another proof? or accountant letter head sufficient for the proof of its regulation body.
