Question regarding Set 0

Hi All,
I will be applying for ILR soon. Is it ok to fill my dependant details in a single form (SET0 ) or should i be using a seperate from for each dependant ?
1.I'm bit confused in filling question no.5.3
5.3 Are you working in the UK? If so, what is your pay each month
after income tax and other deductions ?
I'm working as a contractor through my limited company and the company pays me a minimum salary and also i withdraw dividends . Is it ok to fill such a less amount as a salary ? any ideas ?
2.I'm printing the payslips on a plain paper and i have asked accountant to give a letter stating that payslips are genuine .is this the right way of doing it ?
